LOTO (Lockout/Tagout) is the procedure that neutralizes all hazardous energy in a piece of equipment before work begins: electrical, hydraulic, pneumatic, mechanical, thermal, and stored residual energy.
The trap
The written procedure almost always exists — often 10 to 12 spotless steps in the binder. But at 4 p.m. on a Friday, line down, production manager waiting, the technician short-circuits the sequence: a shared team lock instead of a personal one, no verification of zero energy, no bleeding of residual energy, a tag hung without a lock.
The paper lockout is complete. The field lockout stops halfway. And nothing in the system sees it.
1910.147 ranks among the most-cited OSHA violations year after year. But the real penalty is not the fine: it is the unexpected restart while hands are inside the machine. And after the accident, the perfect binder becomes evidence against you — it proves the procedure existed and was not followed.
How to close the gap (3 actions)
- One lockout procedure per machine. Not a generic “plant LOTO” procedure, but an energy control procedure specific to the machine, listing its real isolation points — attached to the asset record, readable at the machine.
- A blocking “LOTO verified” step. The work order does not move to execution until lockout is confirmed. The checkbox is not decorative: it gates the rest of the workflow.

The permit is issued on a form, signed, filed — then never linked to the work order performed or the equipment involved. An 8-hour hot-work permit sees welding resume the next day on the same paper. The approver signs without seeing the real lockout state. The permit expires — nobody knows.
Hot work is a leading cause of industrial fires (NFPA), and confined spaces kill workers every year who entered without a valid permit. When the incident happens outside the permit window, it is impossible to prove who authorized what: the chain of responsibility collapses at the worst possible moment.

- Link the permit to the WO and the equipment. The permit is no longer an orphan document: it points to the real work order and the asset involved. End-to-end traceability, historized on the machine.
- A timestamped multi-stage approval chain. Who authorizes, at which stage, at what time. Hot work may require sign-off from both the HSE lead and the area supervisor — each link leaves a trace.
- Validity window + automatic expiry. An expired permit blocks resuming the work. No welding tomorrow on yesterday's permit — the system refuses, you must re-authorize.
