Electrical work permit — isolation checklist

Checks to complete on each electrical work permit before work on a de-energised installation: authorised people, every source disconnected and locked, stored energy discharged, absence of voltage verified, earthing, adjacent live parts protected.

This template covers a high-hazard activity. Adapt it to your site’s written procedures, your equipment and local regulations, and have it approved by a competent person before use. When imported, it lands for review: a checklist as a draft, a procedure as inactive.

Category: Permit — pre-work checklist

  • Permit type: Electrical
  • Filled in on each permit concerned, before the work starts.

Imported as a draft: it is not used until you review it and click Activate on the HSE template. Once activated, it is used on every new permit of this type and replaces the checklist in use.

Checklist items

  1. The work is done on a de-energised installation

Live working is not covered by this checklist: it needs its own authorisation and procedure. Live parts left nearby are dealt with below.

Pass / FailCritical: a failure blocks the permit
  2. Voltage level of the installation

    ChoiceRequired
    • Low voltage
    • High voltage
  3. The people doing the isolation and the work are authorised for this installation and voltage level

Pass / FailCritical: a failure blocks the permit
  4. Every source of supply has been identified

Including back-feeds, standby generators, UPS, solar panels, capacitor banks and control circuits fed from elsewhere.

Pass / FailCritical: a failure blocks the permit
  5. Every source is disconnected by its isolating device

A device suited to isolation, with its open position verified, visible where possible. A stop button or a contactor is not an isolation.

Pass / FailCritical: a failure blocks the permit
  6. Every isolating device is locked open and tagged

With the personal padlock of each person working, or through a group lockbox, and a tag giving name, date and reason.

Pass / FailCritical: a failure blocks the permit
  7. Stored energy has been discharged

Capacitors and the DC bus of variable speed drives discharged, after the waiting time given by the manufacturer.

Pass / FailCritical: a failure blocks the permit
  8. The voltage tester has been proven before and after the test

Checked on a known live source or a proving unit, then the absence of voltage is tested, then the tester is checked again.

Pass / FailCritical: a failure blocks the permit
  9. The absence of voltage is verified on every conductor, at the place of work

Pass / FailCritical: a failure blocks the permit
  10. Earthing and short-circuiting applied where required

Always on high voltage; on low voltage where there is a risk of re-energisation or induced voltage. Answer « Pass » if not required.

Pass / FailCritical: a failure blocks the permit
  11. Adjacent live parts are protected or the work zone is marked out

Insulating screens, covers or barriers in place, and the limit of the work zone clear to everyone.

Pass / FailCritical: a failure blocks the permit
  12. Protective equipment and insulated tools suited to the voltage and the arc flash risk are worn during the verification

Pass / FailCritical: a failure blocks the permit
  13. Isolation points and lock numbers

Each isolating device opened and the number of each padlock fitted, so the isolation can be removed in full.
