Customize the request form
Hide fields, rename labels, require a photo
Tailor the intervention request form โ hide fields your requesters don't need, rename labels to match your wording, and require a photo. Hiding fields and renaming labels is a Business plan feature; requiring a photo is available on every plan.
How to customize
- Open Settings โ Intervention Request form.
- Under Hidden fields, check the fields to hide. Under Custom field labels, type your own wording.
- Click Save โ the form updates everywhere, including the public QR-scan form.
Fields you can hide
- Observation option ("Report an observation")
- Priority
- Description
- Type / Category
- Due date
- Requester name
- Requester email
- Requester phone
- Requester company
Rename field labels
Give native fields your own wording โ for example rename "Title" to "Request summary". Leave a label blank to keep the default. Labels show exactly as typed and are not translated.
Require a photo
Make at least one photo mandatory: open Settings โ Intervention Request form, and under Required native fields check Photos. This works on every plan and applies to both the in-app and public QR forms.
Renaming priority & request-type options
On the Business plan you can rename the options shown in the Priority and Request type dropdowns โ for example display 'Critical' as 'Emergency' or 'Low' as 'Routine'. Only the label your team sees changes; the underlying value stays the same, so SLA timers, escalation rules and reports keep working. These labels show as typed for everyone and are not translated.
